    When a person is on Medical Leave rather than FMLA leave, the documents that are required by the U.S. Department of Labor for FMLA are replaced by forms required at the district level for Clay Community Schools that allow us to keep accurate records.  Directly below will be forms that your healthcare provider needs to complete for either your own personal health condition or the health condition of a family member.
